Driver's Permit Information

Please complete THIS GOOGLE FORM to request a School Compliance Verification Form for your NOMS student.  
 
PERMIT FORMS WILL BE AVAILABLE IN THE NOMS OFFICE AFTER 9AM WITHIN TWO DAYS OF COMPLETING THIS FORM DURING NORMAL INSTRUCTIONAL DAYS. You will need to present your ID to ensure we are providing the form to the appropriate person.

*IF YOUR TEST IS SCHEDULED DURING A BREAK OR WHEN SCHOOL IS NOT IN SESSION (FALL BREAK, WINTER BREAK, SPRING BREAK, etc.) PLEASE REQUEST YOUR FORM *AT LEAST THREE DAYS* IN ADVANCE. Staff may not be available to prepare your form during breaks, so requesting early ensures the form will be ready in time for your test.*
